Communicating about transportation at dismissal can be a challenge at the beginning of the year. We are asking that you complete the information below to the best of your knowledge for NEXT SCHOOL YEAR (23-24). IF YOU ALREADY SENT THIS PAPER FORM IN, YOU DO NOT NEED TO COMPLETE THE SURVEY. We will confirm the information in the back-to-school packets we send via mail in August. COMPLETE THIS FORM
